We will ask you to pay for your treatment and will give you signed document to file for reimbursement where it is applicable. If required by your insurance plan, you are responsible for obtaining any referrals or pre-authorizations associated with your appointment. Payment for non-covered services is required at the time of service. We accept all major credit and debit cards, as well as cash.
